In preparing for ‘life after living’, as Anna calls it, palliative care can provide a vital service for people with life limiting illnesses and their families. With the Palliative Care Navigation Pilot program underway, Matt, Anna, and Tim discuss their experiences with grief and loss, and make the effort to have these discussions for the benefit of themselves and everyone around them.
